We provide 24/7 residential care for older adults who need help with daily living, memory care, medication management, mobility support, personal care, and long-term care needs.

  • Fountainhead Homes offers care in smaller, home-like residences with no more than eight residents per home. This allows for more personalized attention, closer relationships between staff and residents, and a calmer environment for individuals who may not do well in a large facility setting. Rather than offering services à la carte, we take a holistic approach to care. Every resident has access to the full range of services we provide as their needs arise, even when those services are not required at the time of admission. Each resident’s care is thoughtfully individualized and adjusted over time to support their comfort, safety, independence, and overall well-being.
